CoCASA Extract Screen |
Purpose |
The Comprehensive Clinic Assessment Software Application (CoCASA) is the CDC software tool for assessing immunization coverage and practices within a provider organization as part of the Assessment, Feedback, Incentives, and eXchange (AFIX) Program.
The CoCasa Extract screen allows users to generate an extract of information from ImmTrac2 to use for CoCASA reports. The CoCASA output is a tab-delimited text file, suitable for importing into the CoCASA application.

The client's birth date. |

The first line of the client's address. |

The second line of the client's address. |
City |
The city of the client's address. |
State |
The state of the client's address. |
Zip |
The zip code of the client's address. |

The organization's Primary Client ID, found in the Update Client screen, under the Organization Information accordion. |
Gender |
The gender of the client. The values are as follows:

Note
ImmTrac2 is set up to accommodate a variety of user types. Each user will have access to certain features in the application based on the role that is assigned.
