Purpose

The Enter NDC screen is used to enter NDC Codes into the system.

Enter NDC Information table - This table displays information that can be entered for a NDC Code as specified in the table below.

Column	Description
Trade Name	This information at the top of the screen displays the Trade Name of the NDC that is being entered.
Vaccine	This information at the top of the screen displays the Vaccine of the Trade Name of the NDC that is being entered.
CVX	This information at the top of the screen displays the CVX Code of the Vaccine of the Trade Name of the NDC that is being entered.
NDC Number	NDC Number is a required text field. Specify the NDC Number being entered in this text box.
Manufacturer	This drop down menu displays all the available manufacturers for the Trade Name associated with the NDC.
Status	Status radio button is required to be selected. Specifying a Status allows the NDC to be specified as displaying on the Receive Order screen.

Public Organizations section - This section will display the NDC information for public organizations for pediatrics, adults and ancillaries.

Column	Description
Orderable	Selecting if the vaccine is orderable for Pediatrics, Adults and Ancillary is required. Select Yes, No, or Out of Stock from the drop down menu.
Dose Size	Dose Size is required to be selected from the Dose Size drop down menu. The drop down menu displays all available Dose Sizes.
Units Per Package	Units Per Package is required to be selected from the Units Per Package drop down menu. The drop down menu displays all available units per package for NDC Codes.
Doses Per Unit	Doses Per Unit is required to be selected from the Doses Per Package drop down menu. The drop down menu displays all available doses per unit available for a unit.
Packaging Description	Packaging Description is a required text field. Specify the Packaging Description of the NDC being entered.

Private Organizations section - This section will display the NDC information for private organizations for pediatrics, adults and ancillaries.

Column	Description
Orderable	Selecting if the vaccine is orderable for Pediatrics, Adults and Ancillary is required. Select Yes, No, or Out of Stock from the drop down menu.
Dose Size	Dose Size is required to be selected from the Dose Size drop down menu. The drop down menu displays all available Dose Sizes.
Units Per Package	Units Per Package is required to be selected from the Units Per Package drop down menu. The drop down menu displays all available units per package for NDC Codes.
Doses Per Unit	Doses Per Unit is required to be selected from the Doses Per Package drop down menu. The drop down menu displays all available doses per unit available for a unit.
Packaging Description	Packaging Description is a required text field. Specify the Packaging Description of the NDC being entered.

Direct Ship Vaccine - Indicator used to identify direct and non-Direct-Shipments.

Column	Description
Direct Ship	Select whether the vaccine is a Direct Ship Vaccine or not.
	Note: When the “Direct-Ship Vaccine” radio button NO is selected, a “Vaccine Cost per Unit" data entry box is displayed for both pediatric and adult
	Note: When the “Direct-Ship Vaccine” radio button Yes is selected, a “Fund Type” selection table will be displayed for the users to choose the type of funding source for the direct NDC vaccine. The user can select the applicable funding sources for the direct ship NDC vaccine.
	If applicable, a dollar limit may be a limiting factor for ordering certain vaccine groups.

Command Button Functionality

Command Button	Description
	Clicking the Save button will save the Entered NDC information if filled out correctly and display the Manage NDC screen.
	Clicking the Cancel button will display the previous screen the user was on.
