Ad hoc List Report

Purpose

The Ad Hoc List Report generates a list of clients based on the criteria selected by the user. The report produces a list with a column for each item, much like a spreadsheet. The information in the report includes clients associated with the user’s organization. The ImmTrac2 User Manual, accessible in the Related Links option of the menu bar, provides a more detailed description of reporting functions.

What items would you like to display on the report? - This section lists client and immunization information details that can be displayed on the Ad Hoc List report. The display items are organized into group categories, and then listed in alphabetical order within each group, as described in the table below.

	White	Race of the Client is White
Address	City	City listed for the Client's Address
	Other address	The second portion of the Client's Address, usually contains apt/suite number information
	PO box	PO box associated with the Client's Address
	Phone number	Phone Number listed on the Client's record
	State	State listed on the Client's Address
	Street	Client's Street Address
	Zip Code	Zip Code listed on the Client's Address
Immunizations	Trade Name	Manufacturer name of the vaccine administered
	Vaccination Date	Date when the vaccine was administered
	Vaccine	Vaccine that was administered
	Vaccine Eligibility	Code used to determine the level of insurance coverage for the vaccine administered
	Vaccine Lot Other Inv	Lot Number of the vaccine administered if not from the ImmTrac Inventory
	Vaccine Group	Vaccine Group of the vaccine that was administered
	Vaccine Lot	Lot Number of the vaccine administered from the ImmTrac2 Inventory
Other Items	Death Date	Date listed on the record for the Client's death
	Death Indicator	Indicates if the client has passed away
	Ethnicity	Client's Ethnicity
	Gender	Client's Gender
	Primary Client ID	Client's identification number in ImmTrac2
Responsible Person	First Name	First Name of the person responsible for the client
	Last Name	Last Name of the person responsible for the client
	Middle Name	Middle Name of the person responsible for the client
	Relationship to client	Relationship of the person listed on the record as being responsible for the client

Display Order - Items within the same group (Client, Address, Immunizations, etc.) will appear in the order they are selected. For example, if the items selected are Last name, Zip code, Birth date, the columns in the report will appear as Last name, Birth date, Zip Code, because last name and birth date belong to the same group (Client) while Zip Code belongs to a different group (Address).

	Adds selected item from the list in the left-hand window to the items in the right-hand window.
	Removes selected item from the list in the right-hand window.
	Removes all items from the list in the right-hand window.

How would you like the report to be sorted? - This section displays a drop-down list of the pre-selected display items so that the user can select an item for sorting the report results. The order can also be defined, as described in the table below.

Sort Order	Description
First-to-Last	Sorts in the following order: spaces, special characters (#,',"), uppercase A-Z, lowercase a-z.
Last-to-First	Sorts in the following order: lowercase z-a, uppercase Z-A, special characters (",',#), spaces.

How would you like to filter the data? - This section allows the user to select from a list of Client and Immunization factors, and make filter selections, to narrow the resulting clients that will display in the report. The factors and filter options are described in the table below.

Command Button Functionality (Filter Options)

Command Button	Description
	Adds filter selections to the Selected Filters box. When existing filters in the box are selected to edit, saves the changes and updates the display in the box.
	Allows users to edit the selected filter.
	Removes the selected filter from the Selected Filter text box.
	Changes the logical operator selected from "AND" to "OR" or from "OR" to "AND".
	Groups together multiple filters selected into one logical statement.
	UnGroups multiple filters selected into separate logical statements.

Generate Report Section

Command Button	Description
	Generates the Ad Hoc List Report and displays the Ad hoc Report Results screen.
	Returns the user to the previous screen.

ImmTrac2 is set up to accommodate a variety of user types. Each user will have access to certain features in the application based on the role that is assigned.
