Immunization History Report Screen

 Purpose

The Immunization History Report screen allows users to generate a group of official immunization history reports for a set of clients at one time. The official immunization history report can also be run for an individual, in the client's record under Client Reports.

Functionality

The Immunization History Report screen displays three check boxes for filtering client records to generate the report. Users may select more than one option. Only clients associated with your organization will be included in the report. Each of the report criteria options are described in the table below.

Option

Description

Birth Date Range

(Default selection) The population for the report will be limited to clients  with a birth date that falls within the entered date range, including clients born on the From or To date. Enter the date in MM/DD/YYYY format or click the calendar icon to select a date.

Site

The population for the report will be limited to clients who had an immunization administered at the specified organization site.

NOTE: Most users will not have other organization site options because their user role defines the client population to be only those clients in the user's organization.

Immunization Date Range

The population for the report will be limited to clients who received an immunization within the entered date range,  including clients who received an immunization on the From or To date. Enter the date in MM/DD/YYYY format or click the calendar icon to select a date.

Sort Order

Sorts the resulting clients in the report by Last Name then Age, or by Age then Last Name.

Command Button

Description

Generates the report and navigates to the History Report Request Status screen.

Clears the criteria selections and returns the screen to its initial state with the first check box selected by default.

Returns the user to the previous screen.
