Student Immunization History Screen |
Purpose |
The Student Immunization History screen displays a student's immunization history and provides vaccine recommendations based on the selected tracking schedule. From this screen, a user can add the student to an existing student report list to run reports for that student.

Student Information Section - This section displays a student’s information as described in the table below.

The student's first name, middle name, and last name. |
DOB |
The student's birth date. |
Gender |
The student's gender. |
Tracking Schedule |
The Immunization Tracking Schedule that is used to determine if a student is up to date on their vaccines. This field displays as a drop-down list containing the ACIP and school schedules. When a schedule is selected, the Vaccines Recommended section refreshes to display the applicable recommendations. |
Current Age |
The student's current age in years, months, and days. Example: 1 year, 2 months, 3 days. |
Reports Section - This section allows the user to add the student to a school report list. A student cannot be added to the same list more than once. After adding a student to a list, the list name no longer displays as an available option in the drop-down list. A student can be added to multiple school lists. Once a student has been added to a list, the name displays in the current report lists.
Immunization Record Section - This section displays all vaccines the student has received to date. Records are listed alphabetically by Vaccine Group, then by date administered, as described in the table below.
NOTE: Any immunization record (row) that appears in the Immunization History table in all red font indicates the client had a reaction to the corresponding immunization.

Vaccine Group |
The vaccine group name for each immunization received. |
Date Administered |
The actual day the client was given the vaccine, as a hyperlink. Click the date to view the "Explanation of Dose Status" pop-up window for that particular immunization. This pop-up window displays the tracking schedule rule for that vaccine series. |
Series |
A sequence number along with the total number of doses within the immunization series displays for valid immunizations. An example is (1 of 4) with the 1 being the dose number of the immunization and the 4 being the total number of doses within the immunization series.
NOTE: NOT VALID will display if the dose given for the immunization series was not considered to be valid within the immunization series tracking schedule rules. |
Vaccine [Trade Name] |
The name of the vaccine provided by the manufacturer (Trade Name). If no Trade Name was provided, the column will only display the type of vaccine that was administered. |
Dose |
The dose magnitude reflects how many doses were administered to the client. This number is not dose volume as measured in milliliters. Here are some examples:
The system uses this data to determine the effectiveness of the vaccination provided. |
Reaction |
Displays an indication of whether a reaction occurred, as a hyperlink. A blank means no reaction was entered in ImmTrac for the vaccine. A 'Yes' means a reaction was recorded for that vaccine. Click the hyperlink to display a small information window with additional details. |
Disaster |
Displays an indication of whether the vaccine was administered during a disaster. A blank means the vaccine was not administered as part of a disaster. A "Yes" means it was administered during a disaster event. Click the hyperlink to display a small information window with additional details. |
Vaccines Recommended Section - Displays the recommended vaccines and corresponding dates based on the selected tracking schedule as described in the table below.

Information in the Vaccines Recommended by Selected Tracking Schedule table may be color coded for emphasis, as defined in the table below.

Explanation of Dose Status Window |
The Explanation of Dose Status Window is a pop-up window that displays after clicking the Date Administered link in the Immunization Record, and displays the tracking schedule rule for that vaccine series. This can aid in explaining why an administered dose was not valid or why a recommendation displays a particular date.
The window displays the schedule for the dose that was selected and includes messages and yellow highlights as part of the explanation of status. For example, to see why a dose of DTaP is marked as "Not Valid", click on the vaccination date for that dose to open a pop-up window that displays the series rules for DTaP. The window displays a comment explaining why the dose was not valid.
Viewing Tips:
The selected dose in the series will be highlighted in yellow.
If a dose is not part of a series in the selected schedule, an applicable message will appear.
If the first dose given for a vaccine group is recorded prior to the minimum age, the same message will display indicating it is not part of the series.
If a client had no immunizations in the series and was too old to complete, the first dose in the series would be highlighted.

ImmTrac2 is set up to accommodate a variety of user types. Each user will have access to certain features in the application based on the role that is assigned.
