Manage Clinicians Screen

 Purpose

The Manage Clinicians screen displays clinicians associated with the organization. These clinicians appear in the Administered By drop down menus during the Add Immunization process.   Users with a Provider Supervisor role can edit certain Clinician Information.

 

Information Provided

The Manage Clinicians screen displays all Clinicians and their designated roles. Clinicians are listed alphabetically by last name. Clicking a name opens the Clinician Information screen.

Columns

Description

Clinician Name

The Clinician's last name, first name as a hyperlink.

Role

The Role of the Clinician.

Signature

Designation of electronic signature on file, if applicable.

Command Button

Description

Navigates to the Clinician Search screen.

Navigates to the Clinician Listing screen.

Edit Clinician Information Screen

Fields

Description

Prefix

Clinician's prefix.

Last Name

Required. Last name of the clinician.

First Name

First name of the clinician.

Middle Name

Middle name of the clinician.

Suffix

Clinician's suffix.

Job Title

Required. Clinician's job title at the organization. **NOTE: Can be edited by a Provider Supervisor.

Specialty

Required. Clinician's specialty practice. **NOTE: Can be edited by a Provider Supervisor.

Employee ID

Clinician's ID number associated with the organization. **NOTE: Can be edited by a Provider Supervisor.

Email

Required. Clinician's email address. **NOTE: Email address can be edited by a Provider Supervisor only when the organization is not a TVFC program provider.

License Type

Credentials and description of the professional license type.

License Number

Required. Unique assigned license number for the designated professional credential. **NOTE: System validates the entry against acceptable format for license type.

NPI

Required. Clinician's NPI number. **NOTE: System validates the entry against acceptable format.

Medicaid Number

Clinician's Medicaid number. **NOTE: System validates the entry against acceptable format.

Provider Activity Status

Clinician's activity status.

Description

Street 1

Clinician's Address.

Street 2

Clinician's additional address information, such as suite #, if needed.

P.O. Box

Clinician's P.O. Box.

City

Clinician's City for address purposes.

State

Clinician's State for address purposes.

Zip

Clinician's Zip code for address purposes.

Email

Clinician's Email address.

Area Code

Clinician's Area Code for the phone number being provided.

Phone Number

Clinician's Phone Number.

Ext

Clinician's Extension at the phone number provided.

Returns the user to the previous screen.

Find Clinician Screen

Fields

Description

Last Name

Searches clinicians whose last name is or starts with the criteria that is entered.

First Name

Searches clinicians whose first name is or starts with the criteria that is entered.

 

Note: It is not required to enter criteria in the search fields.  To get a list of all the clinicians associated with the organization, leave the search fields blank and click Find.

 

Columns

Description

Select

Displays a check box for each clinician. Select the box for the clinicians who will be merged.

Clinician Name

The Clinician's Last Name,  First Name as a hyperlink.  The clinician name is a hyperlink that displays the Edit Clinician Information screen.

Role

The Role of the Clinician.

Active

Displays whether a Clinician is active for the Organization.

    

Command Buttons

Description

Searches for clinicians associated with the organization.

Displays the Clinician Merge screen to combine clinician profiles. Select two or more clinicians from the Search Results table to merge them.

Returns the user to the previous screen.

Clinician Listing Screen

Column

Description

Clinician Name

The name of each Clinician  associated with the Organization. Names display alphabetically, last name first.

Role

The role designated in the Clinician Information screen.

Displays the previous screen.

Clinician Merge Screen

This screen allows the user to combine clinicians. Select at least two clinicians from the Find Clinicians search results table  and click Merge.

Select

A radio button for each clinician. Select the radio button for the clinician who will be kept when the records are merged.

Clinician Name

The Clinician's Last Name,  First Name as a hyperlink.  The clinician name is a hyperlink that displays the Edit Clinician Information screen.

Role

The Role of the Clinician in the organization.

Active

Displays whether a Clinician is active for the Organization.

Command Buttons

Description

Keeps the selected clinician in the merge process.

Returns the user to the Manage Clinicians  screen.
