Reminder Request Output Options Screen

 Purpose

The Reminder Request Output Options screen allows users to customize and generate each of the output options for producing the Client Reminders, and allows users to view the step-by-step criteria evaluation process for generating the Reminder/Recall Report results.

Column

Description

Output

The types of output files that can be produced. Each option is described in detail in the table below.

IMPORTANT: Click the hyperlinked output option name to generate that output file. The resulting output file can be retrieved on the Reminder Request Status screen.

Description of the output option.

Additional Input

Displays options for including additional information for the output file.

  • Duplex Printing: Select the check box to generate the output file with a blank page between each notice page.

  • Report Name: Create a name to describe the output file.

  • Free text: Enter text that appears as a customized "closing" message in the output file. Up to 400 characters can be entered.

  • Phone #: Enter the  phone number that appears in the "closing" message of the output file.

Output Option

Description

Reminder Letter

Produces a standard letter with sufficient room at the top of each page for your organization's letterhead. The body of the letter includes the client's immunization history, recommended immunizations, and space to include the free text and phone number.

Reminder Card

Produces a standard card. The body of the card includes the client's recommended immunizations, and space to include the free text and phone number.

Mailing Labels

Produces mailing labels formatted to print on Avery Mailing Labels #5160, allowing for 30 labels per page.

Client Query Listing

Produces a list of clients who matched the criteria, along with their phone number, full address, and immunization information. Any incomplete or blank lines represent insufficient or missing information.

Extract Client Data

Produces an XML format for the clients who met the criteria that includes the client ID, name, birth date, address, and immunization information.

Client Reminder/Recall Spreadsheet

Produces an Excel spreadsheet listing the client's name, birth date, parent/guardian name, address, phone number, client ID, and immunization information.

Custom Letter

Displays each of the saved custom letters created through the Manage Custom Letter screen and produces the custom letters for the clients who matched the criteria.

Command Button

Description

Navigates to the Preview Clients screen.

Sets the last date a Reminder/Recall notice was sent for all the clients who met the criteria for the Reminder Recall report. This is an internal counter for the system to identify who has been sent  a notice. The date displays on the client record, under the Organization Information.

Sets the last date a Reminder/Recall notice was sent for all clients that met the criteria for the Reminder Recall report that have been immunized by the organization running the report. This is an internal counter for the system to identify who has been sent  a notice. The date displays on the client record, under the Organization Information.

Returns the user to the previous screen.
