Responsible Persons Section |
Purpose |
The Responsible Persons section allows users to view and manage listings of parents or guardians for the client. For example, this is where the user would identify the legal guardian or custodial parent, as needed. Users must be in Edit Client mode to see the command buttons for this section and to make changes to information.

Responsible Person Listing Section - This section displays a table listing the existing entries for the client in alphabetical order. If a "Primary" person has been identified, their information will display first in the table. If a Responsible Person has not been entered, this section will be blank.

Review |
Displays a radio button for each listed person for this client. Selecting the radio button allows the user to review the details for that person. |
Remove |
Displays a check box for each listed person for this client. Selecting the check box allows the user to remove the person from the listing. |
Name |
The responsible person's name, displaying in alphabetical order by last name first. Each row displays address information below the name. |
Relationship |
The responsible person's relationship to the client, from a drop-down selection of options. |
Primary |
A radio button option to indicate who is the primary responsible person. |
Details for Responsible Person Section - This section allows users to enter new information, or to update an existing person's information. When the client record initially displays, by default this section will display the information for the first person in the listing table.
NOTE: If the user intends to make a new entry, then the required fields, marked with asterisk, must be entered in order to save the updates.

Last Name |
Required. The responsible person's last name. |
First Name |
Required. The responsible person's first name. |
Middle Name |
The responsible person's middle name. |
Relationship |
Required. The relationship of the responsible person to the client. Click the drop down arrow to select a relationship. |
Phone Number |
The responsible person's phone number. |
Extension |
The responsible person's phone number extension. |
The responsible person's e-mail address. |

Street Address |
The current street address of the responsible person. |
Other Address |
Other address of the responsible person, such as an apartment number. |
City |
The responsible person's city for address purposes. |
State |
The responsible person's state for address purposes. |
Zip |
The responsible person's zip code for address purposes. |
+4 |
The responsible person's 4 digit zip code extension for address purposes. |
Last Updated Section - Displays the date this section was last updated. If the Add New button is clicked, the last updated date disappears until the new updates are fully saved to the client record.

Clears the Details section so that information can be entered for a new Responsible Person. When the button is clicked, the Last Updated date disappears and the following message displays: "Enter details for the new responsible person, then click 'Apply Changes.'" |

When the radio button next to a Responsible Person listing is selected, displays the details in the data fields at the bottom of this section. |

When the check box next to a Responsible Person listing is selected, deletes the record from the Responsible Persons listing table. |

When changes are made to an existing Responsible Person or when a new Responsible Person is added, applies those changes and displays the updates in the listing table. |

Saves all updates made to different sections, or demographic tabs, of the record. |
