Parent/Headquarters Info

Purpose

The Parent/Headquarters Info tab captures whether the organization is a parent organization, mobile unit or a satellite organization.

Select 'Yes', if the organization is a sub-office, mobile unit or satellite office.
Select 'No', if the organization is a Parent organization.

When the user selects 'Yes' additional information must be provided for the organization.

Required Fields

Field Name	Description
Is this organization a sub-office, mobile unit or satellite office?	Required. Enter 'Yes' or 'No'. When 'Yes' is selected, specify the type of organization from the drop down menu.
Parent Organization's TXIIS ID	Required. Enter the parent organization's TXIIS ID.
	Search for the parent organization details once the TXIIS ID is entered.
Parent Organization's Name	Displays the parent organization's name from the records.
Parent Organization's Physical Address	Displays the parent organization's physical address from the records.
Parent Organization's Phone	Displays the parent organization's phone number from the records.
Parent Organization's Fax	Displays the parent organization's fax number from the records.
