User Accounts Info |
Purpose |
The User Accounts Info tab captures the information of the users associated with the organization. You can assign a user as a Provider Supervisor for the organization from here.

Required. The user's first name. |
Middle Name |
The user's middle name. |
Last Name |
Required. The user's last name. |
Phone Number and Ext |
The user's valid phone number and extension. |

Required. The user's email address. |
Current ImmTrac2 Username |
The user's ImmTrac2 username. |
Provider Supervisor |
Required. Select 'Yes' to assign a Provider Supervisor to the organization else select 'No' |
Provider Supervisor Training Completion Date (if applicable) |
The date when the provider supervisor training was completed. Will be required if a Provider Supervisor was assigned to the organization. |
