User Accounts Info

 Purpose

The User Accounts Info tab captures the information of the users associated with the organization. You can assign a user as a Provider Supervisor for the organization from here.

 

 Required Fields

Field Name

Description

First Name

Required. The user's first name.

Middle Name

The user's middle name.

Last Name

Required. The user's last name.

Phone Number and Ext

The user's valid phone number and extension.

Email

Required. The user's email address.

Current ImmTrac2 Username

The user's ImmTrac2 username.

Provider Supervisor

Required. Select 'Yes' to assign a Provider Supervisor to the organization else select 'No'

Provider Supervisor Training Completion Date (if applicable)

The date when the provider supervisor training was completed. Will be required if a Provider Supervisor was assigned to the organization.

+ Add Additional entry

Adds a new account info section to add another user associated with this organization.
