Report/Student List Screen |
Purpose |
The Report/Student List screen allows a user to view students added to a school report list, as well as run reports for the students on the list. From this screen, a user can view a student's immunization history and remove a student from the list.

Reports Available Section - This section lists the reports that can be generated for all students in the displayed list. Each report is described in the table below.

In a new browser tab, displays a PDF file with the name and date of birth for each student on the list sorted alphabetically by last name. |
Official Immunization History List |
A generated PDF file that displays the Official Immunization History of the list of student including name, date of birth and immunization history for each student on the list sorted alphabetically by last name. Clicking this report option displays the Check School Report screen. |
Immunization Due |
A generated PDF file that displays the name, date of birth and all immunization information for each student on the list sorted alphabetically by last name. The report includes the dates when previous doses of a vaccine were administered. See School Immunization Due List Request section below for details on how to configure and generate this report. |
Select Tracking Schedule Section - This section provides a drop-down option field to select which tracking schedule will be used when running a school report for the displayed list.
Client List Section - This section displays the students associated with this list and allows the user to remove students from the list. The Client List table is described below.

Last Name |
The student’s last name as a hyperlink. Clicking the name displays the Student Immunization History screen for the client. |

Command Button Functionality

School Immunization Due List Request |
Allows the user to enter criteria for the Immunization Due report for the applicable school list the user had previously selected. The criteria for this report is explained below.
Select the Vaccine Group to Report on - Allows a user to choose vaccine groups to report on, based on the previously selected tracking schedule. If a student is overdue for any of the selected vaccine groups according to the selected tracking schedule, then the student will be included in the report, given all other criteria are met. The two radio button options are described below.
Select the Use All Vaccine Groups option to include all the applicable vaccine groups.
Select the Use Vaccine Groups Selected option to choose the vaccine groups to filter for in the report. Scroll through the vaccine group list and either double-click on the desired vaccine group name, or click on the vaccine group name and then click the Add button. All selected vaccine groups will appear in the right list box.
To remove a vaccine group from the report criteria, either double-click the selected vaccine group name in the right list box, or click on the selected vaccine group name from the right list box and then click the Remove button.
Enter the Date Criteria - Allows a user to establish a target date range for the report. The target date range will filter for students who are or will be overdue for the selected vaccine groups on a date falling on or between the From and To dates entered. If the target date range is left blank, today's date will be used.
